Nationwide Availability

The GUIDE program is available across all 50 states, ensuring comprehensive support wherever your patients are located. Empath GUIDE serves beneficiaries in several counties across the state of FL.

Respite Care

Eligible beneficiaries can receive up to 80 hours per year of respite care, fully covered by Medicare, to support unpaid caregivers.

Integrated Support

Empath GUIDE offers medication and meal deliveries, in-home safety assessments, and extensive caregiver support and education.

Comprehensive Care Coordination

We provide end-to-end care management, from transitions of care to connecting with local resources, ensuring your patients receive the full spectrum of support.

Empath GUIDE works closely with primary care providers to enroll Medicare beneficiaries in our program. The role of the primary care provider remains unchanged - Empath GUIDE simply adds an extra layer of support for patients. .

Empath GUIDE Eligibility

Who is Eligible?

  • Community-Dwelling Patients: Individuals living at home or within the community who are not residing in a long-term nursing home.
  • Medicare Fee-for-Service Beneficiaries: Patients must be enrolled in Medicare Parts A and B, with Medicare as their primary payer. This includes beneficiares who are dually eligible for Medicare and Medicaid.
  • Confirmed Dementia Diagnosis: Patients must have a dementia diagnosis confirmed by clinician attestation.

Who is Not Eligible?

  • Medicare Advantage Enrollees: Patients enrolled in Medicare Advantage plans, including Special Needs Plands and PACE, are not eligible.
  • Patients in Long-Term Care Facilities: Those residing in long-term nursing homes are not eligible.
  • Hospice Enrollees: Patients currently receiving Medicare hospice benefits are not eligible.
  • Mild Cognitive Impairment (MCI): Patients with a diagnosis of MCI are not eligible.
