What is Empath GUIDE?
The GUIDE Model, which stands for “Guiding an Improved Dementia Experience”, is a program developed by the Centers for Medicare & Medicaid Services (CMS). It aims to provide comprehensive, person-centered care for individuals with dementia and their caregivers. The program focuses on improving the quality of care and reducing the challenges faced by patients and caregivers by offering structured support, coordinated services, and access to specialized care.
